India's national adults and adolescents HIV treatment guidelines document was published in 2007. This document presents a national approach that integrates principles of ART for adults, pregnant women and adolescents. Treatment criteria for adults and adolescents include CD4 counts of or below 200, CD4 counts below 350 with pregnancy, WHO clinical stage 3 or with TB, or WHO clinical stage 4. The first line regimen ART for adults, adolescents and children are based on the combination of three of these drugs: AZT or d4T, 3TC, EFV or NVP. These guidelines also recommend a second treatment based on LPV/r, ddI and ABC option should first line treatment fail. Provider initiated testing and counseling, ART in pregnancy, ART for HIV-infected IDUs, opioid substitution therapy, nutrition, palliative care, post-exposure and cotrimoxazole prophylaxis, and co-infection with TB and viral hepatitis are also included in this document.
